I need to evoke that kind of sickening reaction that this ad does. They do it by using something familiar (white todler, familiar setting) and use narration to help the twist (child starts scooping water with hands and drinking from toilet bowl) make sense.

Tone is kind of arrogant and makes user feel worse—but works for this. Rhetorical question to viewer ("you won't mind, will ya?)
